ATTENTION PARENTS!

New York State has implemented requirements for physicals, which are required for all students entering pre-kindergarten or kindergarten, grades 1, 3, 5, 7, 9 and 11. Parents of those students impacted by this requirement MUST fill out and submit a Physical Health Examination Form.

  For dental exams for students entering pre-K or K, 1, 3, 5, 7, 9 and 11.

• Health Appraisal Form
