Athletic Physicals

  • SPORTS PHYSICALS FROM YOUR PRIVATE PHYSICIAN WILL NOT BE ACCEPTED. All potential North Tonawanda City School District athletes are required to obtain a sports physical every calendar year. According to the New York State Education Department Commissioner’s Regulations, specifically, CR135.4(c)(7)(i) - “It shall be the duty of the trustees and boards of education … to permit no pupil to participate in such activity without the approval of the school medical officer.” Therefore, the only physical that is accepted by the District is one that is completed by the Board-approved physician(s). To obtain an athletic physical, a student must sign up with the school nurse.

    A pre-participation form must be filled out prior to your scheduled physical. No student will be allowed to try out for a team without a current sports physical by our school's doctor. 


     
    During the school year, all students in grades 7-12 must sign up before or after school at the nurse's office in their building to receive an appointment for a sports physical. Any student interested in joining a sport must have a current sports physical by the district’s doctor before the first day of tryouts. If a student does not have a current sports physical, he or she will not be allowed to try out.

    **NOTE: IF PARENTS/GUARDIANS WISH TO BE PRESENT IN THE EXAM ROOM DURING THE PHYSICAL, THEY WILL HAVE TO ATTEND SPORTS PHYSICALS IN AUGUST.
     

     
    ATHLETIC PHYSICAL FORMS
    A parent/guardian must first complete a Pre-Participation Form before a physical will be administered. This form cannot be filled out until 30 days prior to your scheduled physical. Students can get this form from the nurse.
     
    The Coaches Athletic Placement Process (APP) form is required before you can sign up for a physical with the school nurse. The APP form is available on the Athletic Forms page.  
     
    Physicals are valid for a 12-month period.
     
    All athletes (fall, winter, spring) are encouraged to receive their physical in May or June when sessions are scheduled at NTI, the middle school and the high school. If a student misses a scheduled sports physical, there will be set dates throughout the year made available for those students that missed the May/June physicals. These physicals must be scheduled with the school nurse. To obtain a physical, a student must sign up with the school nurse before or after school. Please call the nurse's office at 807-3611.
