SPORTS PHYSICALS FROM YOUR PRIVATE PHYSICIAN WILL NOW BE ACCEPTED. All potential North Tonawanda City School District athletes are required to obtain a sports physical every calendar year. According to the New York State Education Department Commissioner’s Regulations, specifically, CR135.4(c)(7)(i) - “It shall be the duty of the trustees and board of education … to permit no pupil to participate in such activity without the approval of the school medical officer.” Therefore, the school physician or nurse practitioner retains final approval on any physicals performed by a student’s personal physician. If needed, free physicals by our Board-approved physician(s) will take place during the school year.
Registration of FamilyID must be completed prior to your scheduled physical. No student will be allowed to try out for a team without a current sports physical on file with the Health Office.
During the school year, if a physical with the school physician is needed, students in grades 7-12 must sign up before or after school at the nurse’s office in their building to receive an appointment for a sports physical. Any student interested in joining a sport must have a current sports physical before the first day of tryouts. If a student does not have a current sports physical, he or she will not be allowed to try out.**NOTE: IF PARENTS/GUARDIANS WISH TO BE PRESENT IN THE EXAM ROOM DURING THE PHYSICAL, THEY WILL HAVE TO ATTEND SPORTS PHYSICALS IN AUGUST.Physicals are valid for a 12-month period.All athletes (fall, winter, spring) are encouraged to receive their physical in May or June when sessions are scheduled at NTI, the Middle School and the High School. If a student misses a scheduled sports physical, there will be set dates throughout the year made available for those students who missed the May/June physicals. These physicals must be scheduled with the school nurse. To obtain a physical, a student must sign up with the school nurse before or after school. Please call the nurse's office at 807-3611. If the physical is obtained through the student’s personal physician, please provide a copy to the Health Office.
