Child Care Centers, Babysitter Bus Form

    Please note: You must submit a new form every year.
     
     
    Bus The District provides transportation to any child attending kindergarten through grade 8 between the school a child legally attends and a New York-licensed daycare center located within the District, provided the center site meets the distance eligibility limits established by the District.
     
    Transportation to an unlicensed child care location (babysitter) is limited to a location within the attendance area of the school the child attends, provided the address meets the District’s distance eligibility requirements.
     
    NOTE: TRANSPORTATION TO/FROM DAYCARE AND/OR BABYSITTER IS TO ONE LOCATION IN THE MORNING AND ONE LOCATION IN AFTERNOON. The babysitting location cannot change throughout the week.

    Deadline to submit the Daycare/Babysitter Bus Form: Prior to the beginning of the school year, this form MUST be received at the Transportation Department on or before AUGUST. 1 so that proper planning can be made and to ensure room is available on the bus. Applications received after August. 1 will result in transportation starting September. 30, 2024. NO EXCEPTIONS! The Transportation Department will contact you as soon as arrangements are made via phone call.
