Health Appraisal Form

Use this form for School physicals for students entering PreK, K, 2, 4, 7, 10. The district will also accept completed private physician forms.

Pre-Participation Physical Evaluation Form
(This form is available in the Nurse's Office)

Use this form for Athletic physicals.
A parent/guardian must first complete a Pre-Participation form before a physical will be administered.  This form cannot be filled out until 30 days prior to your scheduled physical. When your child signs up for their physical, with the school nurse, they will then be given an appointment date and time.
